PRESENTER APPLICATION
2009 FLORIDA VENTURE CAPITAL CONFERENCE

Instructions:
There are five different fields on this application where you will be required to submit information:
- Company Info - company name and contact info.
- Management Team - names and contact info.
- Financial Info - highlights from the company income statement and balance sheet for the past three (3) years, current year and two (2) additional years.
- Investor Info - names and contact info.
- Marketing Info - description of the company’s product and/or services, market sector and competitive information.

All applications must be submitted electronically via this web site. Please answer all of the questions in their entirety so that we may be able to evaluate your application. All questions must be answered on this form and should not exceed the allotted length parameters. Incomplete applications or applications with significant missing information will not enable us to evaluate your business, may reduce your chance of being selected and may not be considered by the Committee. No additional information is required for your application to be fully reviewed, however press clippings and any other collateral marketing materials will be accepted and can be attached to this application.
Note:
To submit an application, you will need to create an account with a user ID and password. Your account may be used to submit one application, or if you are a venture capitalist for example, to submit more than one application. Once your account is established, you may login at anytime.
Drafts:
After your account is setup, you may start a draft of your application, "save" and exit the application as often as needed. You may modify the information up until the time you submit your final application. Once you submit a final application, you will receive a tracking number as proof your application was submitted.
Red fields marked with an (*) asterisk, require a completed entry or form will not be processed. Presenters must be pre-approved by the Committee prior to the Conference. A submitted application does not guarantee you will present at the Conference. Acceptance is not automatic.
